Assisted Living Coordinator; LPN

Position Overview

Assisted Living Coordinator (LPN) at Trilogy Health Services, LLC. Coordinate and address health concerns of Assisted Living Residents including assessments, medication/pharmacy management, and daily living activities.

• Collaborate with the Director of Nursing (DON) and Assistant Director of Nursing (ADON) for daily clinical operations.
• Ensure pre‑admission screenings are completed prior to move‑in.
• Direct Lifestyle and Care Needs Assessments to determine initial and ongoing medical and personal care requirements.
• Assist residents with activities of daily living (ADL) and medication administration.
• Facilitate Assisted Living Resident Meetings and follow up on feedback.
• Serve as lead nurse for assisted living residents and participate in on‑call rotation as designated by the DON.

• Active, unencumbered LPN license in the state of Indiana.
• CPR certification.
• Previous assisted living experience and leadership experience preferred.
