Location: Indianapolis
Overview
The Certified Family Nurse Practitioner provides compassionate, patient-centered primary care under physician supervision.
Responsibilities- Assess, diagnose, and manage patients across all ages, focusing on wellness, acute illness, and chronic disease management.
- Collaborate with a multidisciplinary team to ensure holistic care and coordinate services.
- Maintain accurate documentation in the electronic health record and participate in quality improvement initiatives.
- Support health promotion and disease prevention, especially in underserved communities facing social barriers.
Requirements: Certified Family Nurse Practitioner license and 1 year of experience preferred in primary, HIV, or community health.
