Registered Nurse (RN) & Licensed Practical Nurse (LPN) at Creekside Health and Rehabilitation

Licensed Practical Nurse (LPN) Primary Responsibilities
  • Provide daily care, health needs, safety and comfort for residents.
  • Conduct professional assessments and document resident health.
  • Assist with management of nursing unit staff.
Licensed Practical Nurse (LPN) Essential Functions
  • Supervise, perform, and provide written/verbal care assignments, routine skills and procedures to ensure quality of care.
  • Participate in development and implementation of resident care plans, maintain up-to-date plans, document progress, and recommend revisions according to policy.
  • Make daily and periodic visits to observe for symptoms, medication reactions, and treatments.
  • Obtain and document vital signs, intake/output, weights, diet supplements, cultures, etc.
  • Assist with orientation and ongoing training of staff to maintain required skills.
  • Report and document resident or visitor incidents, accidents, complaints, or allegations using the designated system.
Licensed Practical Nurse (LPN) Qualifications
  • Graduate of an accredited school of nursing and holds a valid LPN license in good standing per state and federal requirements.
  • Thorough understanding of safe effective nursing practices in Indiana.
  • Basic computer skills with knowledge of Excel, Word, etc.
  • Personal integrity and ability to deal tactfully with residents, families, visitors, agencies, and personnel.
