- Complete an initial, comprehensive, and ongoing assessment of patient and family to determine hospice needs. Provide a complete physical assessment and history of current and previous illnesses.
- Complete initial evaluations, consents, and admissions for patients.
- Coordinate patient care for assigned caseload.
- Provide professional nursing care by utilizing all elements of the nursing process.
- Assess and evaluate patient’s status.
- Write and initiate plan of care.
- Re‑evaluate patient and family/caregiver needs regularly.
- Participate in revising the plan of care as necessary.
- Initiate the plan of care and make necessary revisions as patient status and needs change.
- Use health assessment data to determine nursing diagnosis.
- Develop a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Include the patient and the family in the planning process.
- Initiate appropriate preventive and rehabilitative nursing procedures. Administer medications and treatments as prescribed by the physician in the physician’s plan of care.
- Counsel the patient and family in meeting nursing and related needs.
- Provide health care instructions to the patient as appropriate per assessment and plan.
- Assist the patient with activities of daily living and facilitate the patient’s efforts toward self‑sufficiency and optional comfort care.
- Must complete weekly visit minimum as established by Office Administrator.
- Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice.
- Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
- Work closely with patients on a 1‑1 level but get strong support from a patient care team.
- Can work independently and on your own from a day‑to‑day basis with activities which include phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations.
- Time management skills to manage visits, documentation, meetings (i.e. to scheduled visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns)
- Ability to learn an electronic medical records system.
- Must be willing to drive with reliable transportation, valid driver’s license, and auto insurance, and be able to travel across the service area.
