NP/PA - Advance Practice Primary Care Provider (Full-Time)

OverviewThe Advance Practice Nurse Practitioner or Physician Assistant will provide Home‑Based Primary Care in patient homes or Assisted Living communities. They hold a panel of patients and are responsible for diagnosing, treating and managing their acute and chronic medical conditions with MD oversight.
Duties & Responsibilities- Perform comprehensive in‑home assessments and assist the primary care team in managing chronic diseases.
- Review symptoms, identify diagnoses and manage active medical treatment.
- Complete physical examinations, medication reviews, vital checks, lab draws, wound care and depression screenings.
- Communicate assessment findings to inform the PCP of potential gaps in care.
- Achieve high quality metrics, risk capture and closure rates.
- Maintain high patient satisfaction and improved health status, including reduced hospitalization and ER visits.
- Think critically to address patient needs, provide resources and follow‑up care to improve stability.
- Educate patients on disease processes, medication and compliance.
- Comply with all HIPAA regulations and maintain security of protected health information.
- Travel 5 days a week to provide care to patients in their homes based on business or market needs.
- Education:
FNP/Family Nurse Practitioner, APRN, Nurse Practitioner or Physician Assistant/PA. - Experience:
- Minimum 2 years of clinical experience as an NP/PA in primary care or home care setting.
- Unrestricted NP or PA license in the State of IN.
- Nurse Practitioner certification from ANCC/AANP.
- Working knowledge of chronic care management and medication options.
- Current DEA licensure / prescriptive authority post‑hire.
- Experience in Family Medicine, Internal Medicine, Gerontology and Palliative Care.
- Certified in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS).
Excellent verbal, written communication and presentation skills; ability to clearly articulate and present concepts and models in an accessible manner to Grace at Home’s team, investors, partners and other stakeholders. Proficiency in all Microsoft Office applications.
