Key Responsibilities Staff Leadership & DevelopmentHiring/Firing , onboarding, orientation/training, mentoring and scheduling nursing staff; conduct performance reviews. Coordinate and scheules the daily workload of wach district utilizing agency personnel to their fullest capacity. Provides daily supervision of district nurses and Home Health Aides to ensure the quality of services and maintence of standards. Participates in commnity related functions as necessary. Sees patients in the home setting as needed.
Reviews each nurses schedule for recertification, discharges and aide supervisory visits. Assist with the orientation of new professional staff to familiarize them with the agency. Directs staff towards appropriate use of community resources. Performs other duties as directed by the Adminstrator or the Director of Nurse. Fills out the patient's re-hospitalization referral/admission form sheet daily and submits it to the Intake department with rcertification.

Ensuring high-quality patient care, developing care plans, managing patient flow, and responding to emergencies.
Job ConditionsPrimarily a desk job which involves minimal and occasional lifting of medical supplues and charts. On occasion, one may be required to bend, stoop, reach and move patient weight up to 250 lbs lift and/or carry up 30 pounds. Travel is required to and from patient's home when supervisor of care is scheduled. The ability to commmunicate, well both verbally and in writing in English is required.
The ability to access patients homw which may not be routinely wheelchair access bile is required. Hearing, wyesight and physical dexterity must be sufficent to perform a phyaical assessement of the patients condition and to perform patient care.
Use of multi-line telephone, calculator and medical nursing equipment, i.e glucometer, etc.
QualificationsMust be a graduate of an accredited School of Nursing. Must be a Registered Nurse licensef in the state of Indiana. A minimum of two (2) years of experience as a professional nurse in a Home Health Agency is preferred. Must have a criminal background check. Must have a current CPR certification.
