Community Health RNsare responsible for delivering medical care to individuals within designated communities, promoting health education, and supporting disease prevention efforts. They administer vaccines and medications, and provide guidance on preventive care, nutrition, fitness, and hygiene. The role includes assessing health needs and risks at both individual and population levels, offering advocacy and connecting patients to essential resources.

RNs in this role must document patient care accurately, comply with public health regulations and professional standards, and demonstrate strong interpersonal skills to engage effectively with diverse populations. Organizational skills are essential for managing multiple patients, maintaining records, and coordinating community health initiatives. Problem-solving abilities are key to identifying health challenges and implementing effective interventions.

This position requires travel to various community settings—including homes, schools, and community centers—as well as performing office-based duties. A valid driver’s license and reliable transportation are required.

Schedule:

Monday through Friday with a flexible schedule; occasional evenings and weekends may be required. No holiday shifts.

  • Requires an Associates of Nursing (ASN).
  • Bachelors of Nursing (BSN) preferred.
  • Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license.
  • Requires that the RN has graduated from a nationally accredited nursing program.
  • Requires Basic Life Support certification through the AHA. Other advanced life support certifications may be required per unit/department specialty according to patient care policies.
