Health Educator – Elevance Health – Indianapolis, IN.
Fully remote, full‑time. Hybrid in‑person training sessions are required only for onboarding and development.
Work hours:
Monday–Friday, 2:30–11:00 PM EST.
- Provide information to members eligible for disease management or medication alert programs.
- Make outbound calls to program members to deliver health information according to program standards.
- Contact members who have not enrolled in disease management to encourage enrollment in the correct program.
- Address situations with members resistant to health coaching and/or disease management interventions.
- Offer consultation to associate colleagues on methods to help members recognize and overcome health barriers.
- Assist in increasing participant motivation by addressing barriers and assessing needs, focusing on incremental behavior change leading to healthier outcomes.
- Conduct behavioral assessments to identify individual member knowledge, skills, and behavioral needs; coordinate with the multidisciplinary clinical team as needed.
- BA/BS in an appropriate field and minimum of 2 years of related experience in direct care health education, exercise instruction, or patient education; or an equivalent combination of education and experience.
- Prior experience in health coaching, disease management in a call‑center environment, and/or patient education and behavior change techniques; strong understanding of disease management principles.
- Certification or advanced certification and/or license appropriate to field of specialty (preferred).
