Job DescriptionWe are searching for individuals who are compassionate, serve with a purpose, and are dedicated to supporting our customers and their team. As a part of our team, this position continuously provides the best care and service to not only our patients, but our customers, colleagues, and the communities that we serve. Serves as a bridge between the community and the health care, government and social service systems.
Assists individuals, families, groups and communities access resources, including health insurance, food, housing, quality care and health information. Provides informal counseling, support, and follow-up.
Hours:
Mon - Fri 8:00 - 4:30 with some Holidays, Weekends, and Evenings

- High School Diploma or equivalent required.
- 0-3 years of experience required.
- Requires basic computer skills.
- Requires knowledge of medical, community, and social resources.
