The Innovative Healthcare Collaborative of Indiana (IHCI) is a joint venture between Community Health Network and Deaconess Health System that supports strategic transformation of the health system. This role is primarily REMOTE with occasional physician office‑based duties. Patient coordination will be conducted over the phone.
Responsibilities- Provide direct social work care management to high‑risk patient populations.
- Deliver psychosocial, environmental, and financial consultations to the integrated care team.
- Engage patients and caregivers in developing active care plans, focusing on behavioral, clinical, social and environmental needs.
- Connect patients with primary care, behavioral health, respite and other community‑based services.
- Maintain accurate and consistent data capture in the EMR and serve as liaison with the IHCI RN Care Advisor when needed.
- Develop and manage a comprehensive inventory of local community resources, including housing, medication assistance, financial and insurance support.
- Conduct psychosocial, environmental and financial assessments to determine specific social needs and make referrals with follow‑up.
- Advocate for patients using outreach strategies to engage diverse populations.
- Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) in the State of Indiana.
- Bachelor’s degree in Social Work required;
Master’s degree preferred. - Minimum of 2 years of experience in population health or value‑based care program settings.
- Minimum of 3 years of experience providing patient‑centered outreach, behavioral health services, needs assessment and support.
- Proficiency in Microsoft Office suite and prior experience using electronic health records for data capture, mining and reporting.
- Knowledge of Medicare, Medicaid and third‑party payer guidelines for community resources.
- Strong communication skills, collaborative teamwork, and a positive problem‑solving attitude.
