Home Health Transitional Aide

Overview

Join as a Home Health Transitional Aide at Independence Home Health LLC
. This role focuses on providing compassionate care to clients in home health settings, accommodating flexible scheduling including overnight and weekend hours.

• Assist clients with ADLs such as bathing, dressing, grooming, toileting, and mobility.
• Provide companionship and emotional support.
• Prepare and feed meals, ensure hydration.
• Perform light housekeeping (laundry, dishes, tidying client areas).
• Assist with safe transfers and ambulation using proper techniques.
• Observe, report, and document changes in client condition to supervising nurse or office staff.
• Follow individualized care plans and agency policies.
• Support transitions of care (hospital to home, facility to home, etc.).
• Maintain a safe, clean, and comfortable environment for clients.
• Respect client privacy, dignity, and confidentiality.
• Communicate effectively with clients, families, and care team.

• Must meet Home Health Aide (HHA) / CNA requirements and hold a valid Home Health / CNA license.
• Pass a background check and drug screening.
• Flexible with scheduling, including overnights and weekend availability.
• Compassionate, reliable, and dependable.
• Preferred: previous home health or caregiving experience.
