• Provide assistance with activities of daily living, including eating, meal planning, bathing, dressing, hygiene/toileting, etc.
• Provide assistance with light housekeeping needs that are directly related to the health and well‑being of the eligible individual, such as bed‑making, dusting, vacuuming, etc.
• Provide assistance with accessing community resources, shopping and appointments.
• Accurately complete and submit Daily Activity Logs and Contact Summaries in a timely manner.
• Adhere to safe transportation procedures.
• Follow appropriate protocol for completion of I/A and Serious Incident reports.
• Maintain flexibility in schedule to meet the family’s needs.
• Attend trainings/in‑services as requested and keep personnel update information current.
• Follow the Activity Plan and/or family instructions regarding personal care of the eligible individual.
• Keep supervisor abreast of pertinent information relevant to the provision of services.
• Follow all safety policies and worker’s compensation program guidelines.

Perform other functions and tasks as assigned.

High School Diploma or equivalent required. Must be at least 21 years of age.

• Must have a valid driver’s license.
