Position Summary

The Sisters Together Community Health Worker will play a key role in promoting health and wellness among women in the Indianapolis community by focusing on prevention, diabetes management, blood pressure control, and physical activity.

This role requires a person who is comfortable working independently as well as part of a team, showing initiative, and using creative strategies to connect with the Indianapolis community and spread the word about the program and organization. They will be expected to work with the CWUW staff and to seek out and build important community partnerships to ensure we are serving women where they live, work, and play, and collaborating with the neighborhoods we serve.

The person stepping into this role should be certified (or willing to be certified) as a Community Health Worker (CHW), and will work closely with women of all ages, backgrounds and abilities to lead movement activities, provide educational workshops, connect women with resources for healthcare and basic needs, and offer ongoing support and follow up to help them achieve healthier lifestyles.

This position is ideal for someone passionate about their community's health and well-being who wants to make a positive impact at the grassroots level.

Please note:

This is NOT a remote position. Candidates should live in the Indianapolis area and will be expected to connect in person with the communities and neighborhoods we serve. Some evenings and weekends.

Qualifications
  • Education: Some college coursework in public/community health, social work, or a related field is preferred, but not required.
  • Certifications: A Community Health Worker (CHW) certification is required as a part of this role. If you do not have this certification, you must be willing to obtain it.
  • Experience: Previous experience in community engagement and outreach, health education, or working with underserved populations is required. Bilingual a plus.
  • Physical fitness: Ability to lead some physical activities like walking programs, stretching, and light strength training. Certifications in group fitness, yoga, or other fitness formats are a plus.
Skills
  • Strong connections within the Indianapolis community, partnership-building experience, and familiarity with local resources. (highly desirable)
  • Strong interpersonal and communication skills, and the ability to engage diverse populations.
  • Basic understanding of chronic conditions like diabetes and hypertension, and the role of physical activity and nutrition in health outcomes.
  • Ability to work independently and as part of a team.
  • Organizational skills and attention to detail in maintaining records and documentation. Grant reporting experience is a major plus.
  • Experience with Google Workspace, Squarespace, Canva, Zoom
Duties and Responsibilities
Outreach and Education
  • Conduct outreach to community members, providing education on the 7 Dimensions of Wellness, diabetes prevention, blood pressure management, and the importance of physical activity.
  • Distribute educational materials and resources related to healthy eating, exercise, and chronic disease management in community spaces.
  • Facilitate group workshops and one-on-one sessions, both virtually and on-site at partner organizations/sites, to educate Indianapolis women about their health and well-being.
Client Support
  • Assist clients in setting personal health goals and developing SMART action plans to achieve them.
  • Provide ongoing support and follow-up to ensure clients are adhering to their health plans.
  • Help clients navigate healthcare systems and connect them with appropriate resources, such as medical providers, nutritionists, and fitness programs.
Monitoring and Reporting
  • Track and document client progress in managing their diabetes, blood pressure, and physical activity levels.
  • Maintain accurate and up-to-date records of client interactions and outcomes.
  • Report on program outcomes to funders and provide supervisors with feedback on potential improvements.
Community Engagement
  • Participate in community events, health fairs, and other outreach activities to promote program services and encourage community involvement.
  • Build relationships with community organizations, local businesses, and healthcare providers to enhance program outreach and resource availability.
Advocacy and Referral
  • Advocate for clients' needs within the healthcare system and community services.
  • Refer clients to appropriate healthcare services, social services, and community resources as needed.
