Outpatient Physical Therapist

We are looking for a full‑time Outpatient Physical Therapist to join our rehabilitation team. Your role will be to plan, implement and evaluate patient care programs designed to increase, maintain or restore function, minimize or prevent disability, and assist patients in reaching their maximum performance levels.

• Evaluate patients and gather appropriate information to determine accurate clinical diagnoses and treatment plans.
• Establish appropriate, functional, and objective goals based upon clinical examination.
• Devise detailed treatment and care plans for patients.
• Provide exceptional patient care and service.
• Maintain friendly, cordial relations with all clients and employees; maintain a positive work atmosphere by acting and communicating in a manner that results in positive and respectful working relationships.
• Cultivate relationships with patients and the community.
• Provide continuous communication with referring physicians and other referral sources to ensure a consistent level of care.
• Maintain patient charts, clinical logs and checklists necessary for compliance.
• Maintain patient confidentiality regarding medical records per company policies and procedures and assure compliance with all HIPAA guidelines.
• Communicate well in a professional setting.
• Perform additional duties as required.

• Bachelor’s, Master’s or Doctorate Degree from an accredited Physical Therapy Program.
• Current State of Indiana Physical Therapy license (or willingness to apply).
• CPR Certified.

Pay Range: $80,000 – $100,000 annualized. Base pay is only a portion of the total rewards package. Compensation is determined by job‑related skills, level of experience, education, certifications, geographic location, market data and internal equity.
