Come join the SSM Hospice team! We are looking for a full time Hospice RN that is compassionate, knowledgeable, and willing to travel to our nine counties if needed. You will work 80 hour in a 2 week time frame and call is a requirement as well as some weekend work.
This lead will work in the office but also in the field when needed.
This position covers 9 counties with Jefferson being the center point. Most of staff live within 45 miles or so but distance is not a qualifier.

$34.41 - $55.06

Job Summary:
Provides leadership and coordination of clinician/patient staffing and/or scheduling while ensuring implementation of clinical objectives and quality patient care to patients within a home setting or an SSM hospice house location.
Job Responsibilities and Requirements:
PRIMARY RESPONSIBILITIES
• Assigns, leads, educates and monitors nursing and support staff during assigned shift. Contributes to or performs performance evaluations of staff.
• Functions in the role of supervisor or branch manager in branch leader absence and takes a lead role in process improvement responsibilities. Assists branch leaders in preparing clinical reports and operational duties.
• Assesses daily patient care needs and develops and distributes patient care assignments, ensuring that population appropriate patient care is given. Performs direct patient care as needed.
• Coaches staff on effective and efficient case management & collaborates with other health care providers to deliver results for positive clinical & financial outcomes.
• Collaborates with advanced practice providers to ensure quality outcomes.
• Evaluates the effectiveness of patient care and identifies any issues that affect desired patient outcomes.
• Responds to complaints about patient care and manages through established channels.
• Performs ongoing chart review on case load with attention to: a) Accurateness of 485, recertification, verbal orders for changes in Plan of Care. b) Assuring every visit performed is within assigned frequency. c) Assuring overall documentation supports coverage of service as defined by regulation or payor.
• May provide input for hiring, training, directing, developing and evaluating staff.
• Works in a constant state of alertness and safe manner.
• Performs other duties as assigned.
EDUCATION
• Graduate of accredited school of nursing or education equivalency for licensing
EXPERIENCE
• One year of registered nurse experience
ADDITIONAL REQUIREMENTS
• Proof of motor vehicle liability insurance that meets minimum state requirements upon hire and upon request.
PHYSICAL REQUIREMENTS
• Constant use of speech to share information through oral communication.
• Constant standing and walking.
• Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
• Frequent sitting, reaching and keyboard use/data entry.
• Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
• Frequent use of smell to detect/recognize odors.
• Frequent use of hearing to receive oral communication, distinguish body sounds and/or hear alarms, malfunctioning machinery, etc.
• Occasional lifting/carrying and pushing/pulling objects weighing lbs.
• Occasional lifting/moving of patients.
• Occasional bending, stooping, kneeling, squatting, twisting, gripping and repetitive foot/leg and hand/arm movements.
• Frequent driving.
• Rare crawling and running.
