Job Description- Compiles data, tracks outcomes, and makes recommendations for process improvement.
- Develops and maintains relationships with physicians, post‑acute providers, therapists, patients, and families.
- Collaborates with care management, physicians, and clinicians to develop discharge plans requiring post‑acute services.
- Ensures seamless transitions across the continuum of care.
- Educates patients and families about post‑acute care services and expectations.
