OverviewPlans and manages patient care according to each patient's needs. Interviews patients and records their medical history and physical condition. Obtains patient vital signs, including pulse, blood pressure, temperature, and respiration. Provides routine care for patients.

Responsibilities- Provide 1:1 care for client following client’s plan of care while maintaining client’s dignity and independence.
- Monitor clients’ vital statistics and report abnormal findings to the case manager and physician.
- Perform medication administration, feedings, oxygen, ostomy care as needed per client’s plan of care and provide assessment results.
- Complete routines as established by other care professionals such as Physical Therapist, Occupational Therapist, and Speech Therapist.
- Ensure infection control policies are maintained at all times, report possible communicable diseases or infections, and implement infection control procedures.
- Accurately document nursing actions of all care given and communicate with family, case manager, and physicians, documenting progress and outcomes for established goals and informing about changes in client’s condition and needs.
- Order supplies and medications as needed to ensure adequate amounts are on hand at all times.
- Ensure all patient needs are met under the instructions of the physician.
- Monitor incidents involving individuals and ensure incident reports are completed timely and accurately; inform case managers and examine the individual following the incident.
- Ensure all medical equipment is maintained and report faulty equipment for repair.
- Perform other duties as assigned.
- Completed post‑secondary coursework to facilitate licensure as a Licensed Practical Nurse or Licensed Vocational Nurse.
- Knowledge of individual service plans, including understanding of client rights and health needs.
- Proof of current license as a Licensed Practical Nurse or Licensed Vocational Nurse in good standing from the state(s) of practice.
- CPR certified.
- Excellent interpersonal skills, including customer service, patience, empathy, and compassion.
- In‑depth understanding of patient care policies and regulations.
- Familiarity with symptoms and signs of improvement for several medical conditions.
- Moderate amount of travel between facilities may be required.
