Social Services / Admissions Director (Dual Role)

Join a team dedicated to compassionate, individualized care. Our facility is seeking a Social Services / Admissions Director to support residents throughout their stay and lead our admissions process with professionalism and empathy.

This dual-position oversees resident psychosocial needs, care planning, and discharge coordination, while also managing referrals, tours, admissions, and census updates. You’ll work closely with nursing, therapy, and the full interdisciplinary team to ensure smooth transitions and exceptional resident experiences.

• Bachelor’s Degree in Social Work (BSW)
• 2–3 years’ experience in social services/discharge planning in long-term care or skilled nursing
• Knowledge of Medicare, Medicaid, and insurance processes

• Develop and update care plans
• Participate in care plan meetings
• Complete documentation for admissions/discharges
• Support end-of-life planning
• Coordinate discharge plans with residents and families

• Manage full admissions process
• Conduct facility tours
• Track referrals and communicate updates to the IDT
• Welcome new residents and collect required documentation
• Assist with census, meetings, and room assignments
