Job Description:
* 7 On / 7 Off Nights
* Perform all pharmacist functions including compounding, dispensing, verification, and documentation of medications across central, satellite, and IV areas.
* Review and monitor medication orders and patient profiles to prevent, identify, and resolve drug-related problems; complete pharmacist consults and perform pharmacokinetic dosing, monitoring, and adjustments.
* Provide safe, timely, and cost-effective medication therapy; actively participate in stewardship, dose optimization, surveillance, and restricted-use programs.
* Engage in direct patient care and interdisciplinary rounds, ensuring smooth transitions of care from admission through discharge and follow-up.
* Educate and counsel patients, families, and healthcare team members on safe and effective medication use.

QUALIFICATION REQUIREMENTS:
Bachelor's (Required), Doctorate Advanced Cardiac Life Support Cert (ACLS) - Accredited Issuing Body, Basic Life Support - CPR Cert (BLS) - Accredited Issuing Body, Licensed Pharmacist (RPh) - EV Accredited Issuing Body, Pediatric Advanced Life Support Cert (PALS) - Accredited Issuing Body
