Licensed Clinical Social Worker (LCSW) or Licensed Clinical Professional Counselor (LCPC) – Long-Term Care Facility (Mount Zion Area)
We are searching for a compassionate and dedicated Licensed Clinical Social Worker (LCSW) or Licensed Clinical Professional Counselor (LCPC) to join our team at a long‑term care facility in the Mount Zion area
. This in‑person position focuses on providing psychotherapy and mental health support to residents in a long‑term care setting. The ideal candidate will have strong clinical skills, a collaborative mindset, and a passion for improving the emotional and psychological well‑being of older adults and individuals with complex health needs.
- Conduct comprehensive mental health assessments and develop individualized treatment plans for residents.
- Provide ongoing psychotherapy and counseling using evidence‑based approaches such as CBT, motivational interviewing, and person‑centered therapy.
- Facilitate individual and group therapy sessions to address depression, anxiety, grief, adjustment issues, and other behavioral health concerns.
- Collaborate with facility staff, nursing teams, and physicians to ensure integrated and holistic care.
- Maintain accurate, timely, and compliant documentation, including assessments, progress notes, and treatment updates.
- Participate in care planning meetings and provide behavioral health insights to support residents’ overall well‑being.
- Support residents and families with education, coping strategies, and guidance on available community resources.
- Master’s degree in Social Work (MSW) from an accredited institution.
- Current Illinois LCSW or LCPC license in good standing.
- Experience providing psychotherapy, preferably in long‑term care, hospital, or medical settings.
- Strong understanding of mental health diagnoses, treatment planning, and documentation standards.
- Excellent interpersonal and communication skills, with the ability to work collaboratively in a multidisciplinary environment.
- Familiarity with EMR systems and HIPAA compliance.
- IM+CANS certification is strongly recommended (training available if not yet completed).
