Employment Term:
Maximum Term to 31 January 2027, Part-time (60.8 hours per fortnight)

Your part in our Uniting Care team

The Case Manager for the Minds Alive program provides both intensive long term outreach support and or short-term assistance for children/young people and their families where the child or young person is affected by or at risk of mental illness.

The Case Manager will also provide educational workshops and seminars for different key groups including client groups to promote better outcomes for children or young people that present with mental illness and their families.

Key Responsibilities
  • Conduct eligibility and intake assessments.
  • Undertake comprehensive assessments of risk, protective factors including strengths and the type of support required by the child, young person and their families
  • Work alongside the client to develop their Family Action Plan and support the child, young person with achieving their goals
  • Provide therapeutic, educative and practical support for the child, young person, and their families, connecting them with services and resources in their communities
  • Actively participate with case allocation, case review and community forms, internal/external meetings,
  • Facilitate educational workshops and community based seminars, including developing material and resources
What makes a difference for us?
  • Hold a bachelor’s degree or higher in psychology, Social Work, Behavioural Science or Human Services.
  • Demonstrated skills and sound knowledge in working with children, young people and their families using child-centered and family focused approach.
  • Ability to use therapeutic processes and practices particularly in relation to children and young people affected or at risk of mental illness and their families within frameworks and practices that are strength based and recovery focused.
  • Current Driver License
  • Blue Card
