• Effectively screen, evaluate, and develop plans of care for patients with cognitive and/or ADL physical functioning disorders following all regulatory and clinical practice standards.
• Utilize standardized assessments and evidence‑based practice to support clinical interventions.
• Provide rehabilitative, skilled, and medically necessary treatment interventions, including ADL retraining, therapeutic activities, exercises, modalities, and use of supportive and assistive devices.
• Assess patient progress, adjust treatments, and evaluate outcomes for effectiveness.
• Offer consultation and counseling to patients, families, caregivers, and other service providers.
• Communicate patient progress, barriers, and treatment plans with supervisors and the health team.
• Provide comprehensive discharge summaries per regulatory requirements.
