Position Summary

We are hiring a PRN Mental Health Registered Nurse for the Hurricane office in WV, serving Cabell, Wayne, Putnam, and Kanawha counties. Personal nurses provide care for adult and pediatric patients in their homes. We provide PPE for safety during COVID‑19, and as a PRN Mental Health RN you will deliver patient care, ensure quality, and be an integral part of the interdisciplinary team.

Job Functions / Responsibilities
  • Assessment & Planning
  • Perform initial comprehensive assessment, including OASIS when required by Medicare.
  • Conduct physical, psychiatric, psychosocial, and environmental assessments.
  • Identify mental health diagnoses, symptoms, and risk factors for self‑harm or harm to others.
  • Collaborate with physician and interdisciplinary team to develop individualized Plan of Care (POC).
  • Review and revise POC at least every 60 days or as condition changes.
  • Skilled Nursing & Mental Health Care
  • Provide direct skilled nursing services including medication administration (psychotropic medications), symptom monitoring, wound care, and health teaching.
  • Deliver psychiatric nursing interventions such as crisis stabilization, coping skill development, and relapse prevention.
  • Monitor and manage medication side effects and coordinate adjustments with prescribing provider.
  • Educate patients, caregivers, and families on disease process, medication adherence, symptom management, and resources.
  • Provide suicide risk screening, mental status examinations, behavioral assessments as indicated.
  • Documentation & Compliance
  • Complete and submit documentation—including OASIS—on visit day or within required timeframe to meet Medicare and state requirements.
  • Document all skilled interventions, education, measurable progress toward POC goals.
  • Report changes in patient’s condition to physician and Director of Nursing per CoP 484.60(c).
  • Maintain compliance with HIPAA, Medicare CoPs, and agency policies.
  • Coordination & Communication
  • Participate in interdisciplinary case conferences to review progress, modify care plans, and ensure continuity.
  • Collaborate with social workers, therapists, aides, and other team members to address social, emotional, and environmental needs.
  • Facilitate transitions to and from inpatient psychiatric or medical facilities.
  • Quality & Safety
  • Adhere to infection prevention and control measures per CoP 484.70.
  • Participate in CHHA’s Quality Assessment and Performance Improvement activities.
  • Report incidents, unusual occurrences, safety concerns promptly.
Qualifications Required
  • Current and unencumbered RN license in the state of practice.
  • At least one year of recent nursing experience, including at least six months in psychiatric or mental health nursing.
  • Completion of a psychiatric nursing orientation program approved by the agency.
  • Current CPR/BLS certification.
  • Valid driver’s license, automobile insurance, and reliable transportation.
Preferred
  • Bachelor’s degree in Nursing (BSN).
  • Certification in Psychiatric‑Mental Health Nursing (RN‑BC).
  • Prior home health nursing experience in a Medicare‑certified CHHA.
  • Working knowledge of OASIS data collection and Medicare documentation requirements.
