Job Description

As an Advanced Practice Provider in Family Medicine, you will work with a team of healthcare professionals committed to providing quality care and excellent patient experience. In this role you will work with 4 physicians and 3 advanced practice providers who place high importance on teamwork and collaboration. Our team is well-known for synergy, and high patient satisfaction scores.
- You will practice at Hurricane Valley Clinic in Hurricane, UT
- Your hours will be full-time, and your schedule may be 3 to 4 days per week depending on the clinic's needs. Our clinic is open Monday – Friday 7am – 7pm weekly.
- Your on-call responsibilities are shared among all providers and will be approximately every 7th week for 7 days.

What You’ll Bring- APRN or PA-C degree from an accredited program
- Board certification
- Active, unobstructed Utah medical license, or the ability to obtain one
- Full prescriptive authority and an active DEA, or the ability to obtain one
- Ability to successfully complete Intermountain Health’s credentialing process
- 2+ Years of experience is preferred
- BLS Certification
