Our agency is currently looking for an RN Case Manager to add to our growing team.
DUTIES:- RN to make home visits for new patient hospice admissions, patient assessments, and prn visits. RN will be responsible for collaborating care with patient families, facilities, or personal care homes. Full time RN's rotate on call for 1 week each month.
- Hospice experience is a plus but not required.
- Must have reliable transportation.
