Position Overview:Licensed Practical Nurse (LPN)
We are actively seeking compassionate and skilled Licensed Practical Nurses (LPNs) to join our interdisciplinary healthcare team. In this role, the LPN provides hands-on nursing care, supports rehabilitation goals, and works closely with RNs, physicians, and therapists to ensure residents receive safe, dignified, and high-quality care.

Key Responsibilities of the LPN:
Provide direct patient-centered nursing care in a long-term care and rehabilitation settingAdminister medications and treatments as prescribed by physiciansMonitor residents’ conditions, including vital signs, and promptly report changes to the supervising RN or physicianAssist residents with activities of daily living (ADLs) such as bathing, dressing, and feeding when neededCollaborate with the interdisciplinary care team to support rehabilitation and recovery plansMaintain accurate, timely, and compliant documentation in the electronic medical record (EMR)Provide emotional support and education to residents and their familiesPromote a safe, clean, and therapeutic environment by following infection control protocols🏥 Qualifications for the Licensed Practical Nurse (LPN):
Current and valid Licensed Practical Nurse (LPN) licenseNew grads encouraged to apply!Experience in long-term care, skilled nursing, or rehabilitation preferredStrong clinical skills, including medication administration and basic wound careExcellent communication, documentation, and teamwork skillsCompassionate, patient-focused approach to careBLS/CPR certification required💼 Why Join Our LPN Team?
