Position Overview:Restorative LPN Nurse
The Restorative LPN Nurse assumes daily responsibility and accountability for the restorative nursing care of assigned residents. This role coordinates restorative services, collaborates closely with nursing leadership and therapy departments, and ensures restorative care is delivered in alignment with each resident’s comprehensive assessment and plan of care.

Key Responsibilities of the Restorative LPN:
Oversee and coordinate the restorative aspect of nursing care for assigned residentsConfer with the Director of Nursing Services and licensed nursing staff regarding restorative care needsParticipate in the MDS/RAI process and care planning related to restorative nursing servicesInitiate therapy and evaluation requests and respond to referrals from residents, physicians, or nursing staffServe as a liaison between the nursing department and therapy servicesEvaluate residents and review medical records to identify normal and abnormal findingsRecognize when residents require physician evaluation, supervision, or directionImplement and maintain facility policies and procedures related to restorative nursing careCommunicate restorative care plans and policies to physicians, residents, and family membersUtilize resources efficiently to support optimal resident outcomes🏥 Qualifications for the Restorative LPN Nurse:
Current Licensed Practical Nurse (LPN) license in good standingMust meet all applicable federal and state regulatory requirementsDemonstrated basic nursing skills and clinical knowledge related to elderly and disabled populationsProven leadership and supervisory skillsStrong assessment and communication abilitiesPatient, tactful, enthusiastic, and positive attitude toward residents and familiesPassion for promoting independence and quality of life through restorative care💼 Why Join Our Restorative Nursing Team?
