Job Description

Care Transition Coordinators serve as the primary liaison between the company and referral sources. They develop, maintain, and service community relations in several settings and act as a resource to provide education and information on the company’s programs and services to patients, caregivers, physicians, case managers, discharge planners, social workers, and others. The coordinator is responsible for obtaining patient referrals within the assigned territory for the company.

Job Summary

Reports to Director of Marketing and/or Region Manager.

Qualifications
  • Education
    • Required:

      Associate’s Degree in Business, Nursing, Marketing, or a healthcare‑related field. In lieu of a degree, five or more years of business‑to‑business and/or industry selling experience.
    • Desired:
      Bachelor’s Degree in Business, Nursing, Marketing, or a healthcare‑related field. Formal sales training preferred.
  • Experience
    • Required:

      One year of sales experience. One or more years of related nursing experience may substitute for sales experience.
    • Desired:
      Two to five years of sales experience. One or more years of sales or nursing experience in home health or hospice.
  • Licenses, Certifications, and Registrations
    • Current Alabama driver’s license.
    • Current automobile liability insurance.
    • Reliable transportation and cell phone.
  • Specialized Knowledge and Skills
    • Medicare guidelines for home care services.
    • Communication, public relations, and health education.
    • Basic knowledge of sales.
    • Community assessment relating to geriatric needs.
    • Knowledge of physician, hospital, skilled nursing facility, case management, and discharge planning service needs.
    • Strong communication, problem‑identification, resolution, and strategic planning skills.

    • Effective communication of homecare and hospice services to all levels of the healthcare industry.
    • Ability to accept responsibility for direction, control, and planning of activities.
    • Ability to handle stressful situations and deadlines.
    • Ability to demonstrate strong communication skills and team collaboration.
    • Recognition of rights and responsibilities regarding company and patient confidentiality.
    • Ability to manage multiple assignments and redirect priorities on short notice.
    • Travel within assigned territory and attendance at sales meetings.
Primary Job Functions
  • Manage sales performance and meet monthly home health/hospice budgets.
  • Review territory regularly and discuss with manager to optimize market potential.
  • Host sales meetings to collaborate with operations and achieve company goals.
  • Target and efficiently cover assigned territory weekly, monthly, and quarterly.
  • Develop and implement weekly and monthly call plans for optimal territory productivity.
  • Identify new referral sources and maximize growth opportunities.
  • Maintain knowledge of all referral tracking systems and discuss trend data at meetings.
  • Meet territory‑specific daily and weekly call quotas and maintain accurate assignment reports.
  • Apply strong selling skills, pre‑call planning, and sales process knowledge to build relationships.
  • Use available resources such as sales material, branch staff, medical directors, and marketing budget appropriately.
  • Problem‑solve and resolve conflicts as they arise.
  • Attend meetings promptly and complete all assignments on time.
  • Document sales calls by Monday at 8 am and log expenses and referrals by specified deadlines.
  • Complete timesheets and mileage reports by 9 am on Monday.
Working Conditions

Travel by car to physicians’ offices, hospitals, community centers, and events with exposure to all weather extremes. The environment may be unpredictable and not under the agency’s control.

Physical Requirements
  • Perform limited to full‑range heavy work, including sitting, standing, walking, and driving.
  • Possess vision, hearing, and physical dexterity necessary to maintain a driver’s license and operate an automobile.
  • Climb stairs, access and operate elevators, and maneuver on all terrain.
  • Communicate effectively both orally and in writing.
  • Occasional exertion up to 50 pounds, frequent up to 25 pounds, and constant up to 10 pounds.
