Social Worker - Case Management - FT - 1st Shift

Join Huntsville Hospital as a Full‑time Social Worker in the Case Management team. This role focuses on supporting inpatient patients and coordinating their transition from hospital to home, ensuring continuity of care and optimal use of resources.

• Facilitate resource management activities for inpatients based on identified needs.
• Coordinate patient transitions from hospital to home in collaboration with physicians, nursing staff, and other healthcare professionals.
• Provide psychosocial assessment, resources, and interventions to hospitalized patients.

• BS degree in Social Work from an accredited college or university.
• Current Alabama licensure in Social Work.
• Two years of hospital experience preferred; comparable education and experience will be considered.
• Prior discharge planning or hospital‑based social service experience is highly beneficial.

• Excellent communication skills and ability to work with a diverse population.
