Our Population Health department is searching for a motivated, outgoing, organized individual who is interested in being part of an innovative, team-based approach to healthcare. In this role, the ideal candidate will focus on improving the health status and care for individuals with chronic medical conditions, function as a part of an interdisciplinary team and ensures excellence in patient care.

Under general supervision and according to established policies and procedures,
perform clinical procedures to prepare patients and/or research participants for treatment or research protocol; assist physician or other healthcare team member with completion of treatment/protocol; perform simple, non-invasive patient procedures; may perform various clerical and/or lab duties as requested; respond to the unique needs of all patients ages newborn through geriatric.

• Work with Population Health Nurse supervisor to identify patients for Medicare annual wellness exams (AWV) and diabetic eye exams.
• Track results of screening tests to close care gaps, close referral appointment loop, schedule diagnostic testing.
• Perform AWV exams under direct supervision of the resident or faculty.
• Assist Population Health Nurse supervisor with quality improvement projects.
• Schedule patients for AWV appointments. Accomplish weekly and monthly team goals.
• Demonstrate commitment to providing outstanding customer service in a manner that reflects our mission, values and customer service standards.
• Attend and actively participate in training-related activities as needed.
• Perform other duties as assigned.

$15.00 - $17.00

Huntsville, Alabama

High School diploma or GED required.

Certified in Basic Life Support (BLS) from the American Heart Association (AHA) is required or should be obtained within 30 days of employment. Depending on the unit/department, Advanced Cardiac Life Support (ACLS) certification and/or Pediatric Advanced Life Support (PALS) from the American Heart Association (AHA) may be required.
