In this role, you will assist the Care Facilitator and/or Social Worker with discharge planning responsibilities to ensure timely and safe discharge services. You will facilitate and coordinate discharge services such as home health, DME, Rehab placement, and connect with community resources. Ideal candidates should have experience in discharge planning and strong communication skills.
