We are seeking a compassionate and motivated Restorative Nurse Aide (CNA) to join our healthcare team at the Heritage Pointe of Warren. The Restorative Nurse Aide works under the direction of the Director of Nursing and Therapy Team to assist residents in maintaining and improving their highest level of physical, cognitive, and psychosocial functioning. This role is vital in supporting resident independence and quality of life in a long-term care setting.
Assist residents with restorative nursing programs including range of motion (ROM), ambulation, transfers, positioning, splinting, and adaptive equipment use
Encourage and motivate residents to participate in restorative activities per individualized care plans
Document resident participation, progress, and tolerance of restorative programs accurately and timely
Communicate changes in resident condition or functional ability to the Restorative Nurse or licensed staff
Assist with activities of daily living (ADLs) as needed while promoting resident independence
Follow established policies, procedures, and safety guidelines at all times
Collaborate with nursing, therapy, and interdisciplinary team members
Maintain a clean, safe, and supportive environment for residents
QualificationsPrevious experience in long-term care preferred (restorative experience a plus, but not required)
Ability to follow written and verbal instructions
Strong documentation and communication skills
Compassionate, patient-centered approach to resident care
Ability to lift, push, pull, and assist residents as required
