Job Responsibilities:

• Provide skilled physical therapy care to patients in their homes, as prescribed by physicians.
• Ensure compliance with regulations established by the Indiana State Board of Nursing and agency policies.
• Evaluate and treat patients using therapeutic procedures to restore muscle strength, coordination, and body alignment, relieve pain, and improve movement and function.
• Work collaboratively within a team, supporting and informing other healthcare professionals.
• Provide therapy services in various settings including Skilled Nursing Facilities, Outpatient Facilities, Long Term Care Facilities, or Home Health Care.

Qualifications:

• Bachelor of Science or higher degree in Physical Therapy.
• Current state license/certification and registration with the American Physical Therapy Certification Board.
• Preferably at least one year of rehabilitation experience.
• Excellent communication and interpersonal skills.
• Knowledge of accreditation standards and compliance requirements.
