Overview

The Caseworker II role at Parkview Health provides monitoring and supportive intervention for Park Center's child, adolescent, and adult clients. The position involves communicating progress and problems to the treatment team (including family members), arranging transportation when needed, responding to crises, and providing crisis intervention in consultation with the team. The role supports development and execution of the treatment plan through home visits, community liaisons, and collaboration with school systems as needed.

• Monitor and provide supportive intervention for clients across child, adolescent, and adult populations.
• Communicate progress and issues to treatment team members and family as appropriate.
• Provide and/or arrange transportation services when needed.
• Respond to crisis calls and provide crisis intervention in consultation with the treatment team.
• Participate in the development of the treatment plan for assigned clients.
• Carry out the treatment plan through home visits and community liaisons.
• Coordinate with school systems as needed.

• Bachelor’s degree in social work, psychology, sociology, human services, or related field.
• Valid state driver’s license, reliable transportation, and proof of auto insurance.
• CPR certification required within 30 days of employment.
• At least one year of experience in a mental health environment.
