- Delivery of Patient Care:
Assumes responsibility for the coordination and delivery of patient/caregiver care within contracted facilities. - Conducts nursing assessments to assist in the overall management of patient/caregiver needs, collaborating with facility staff to ensure a seamless care experience.
- Develops, implements, evaluates, and updates the plan of care to ensure care is uninterrupted and hospice goals are integrated with the facility’s daily care routine.
- Facilitates patient and caregiver independence to the extent possible within the facility environment.
- Appropriately utilizes VIA Health Partners patient-related protocols, standing orders, and guidelines while respecting facility-specific safety protocols.
- Assumes responsibility for conducting effective patient, caregiver, and facility staff education regarding hospice philosophy and end-of-life care.
- Encourages and allows time for questions and consultation with both families and facility partners.
- Communicates effectively with members of the IDG, patients, caregivers, and facility healthcare professionals to ensure continuity of care and symptom management.
- Maintains open lines of communication with supervisor, keeping them apprised of complex patient issues or facility-level concerns.
- Applies principles of infection control, following both VIA Health Partners and facility-specific universal precautions.
- (IPU) Cleans patient rooms as required per patient room cleaning procedure. Ensures all areas of facility are clean and appropriately stocked with supplies. Keeps the family room clean throughout the day. Ensures proper chemicals are used for cleaning different surfaces. Ensures protective equipment is used when working with chemicals, i.e., gloves, glasses.
- Attends, participates in, and may facilitate interdisciplinary group conferences (IDG) and special care conferences, ensuring input from facility partners is represented.
- Assumes responsibility for establishing and maintaining effective working relationships with team members and facility staff.
- Identifies specific patient/caregiver needs and makes referrals to appropriate team members.
- Ensures accurate, complete, succinct, and timely clinical documentation in accordance with VIA Health Partners guidelines, ensuring documentation reflects coordination with the host facility.
- Demonstrates competency in knowledge and skills relevant to hospice care in a long-term care or assisted living setting.
- Ensures that VIA Health Partners’ professional reputation is maintained and projected to all external partners.
- Maintains current professional licensure, CPR certification, and CEU requirements.
- Maintains productive and professional relations with external agencies, facility administrators, and onsite healthcare providers.
- Exhibits expert time management skills, efficiently managing a travel route while remaining able to respond appropriately to patient crises.
- A Diploma or associate degree in nursing is required;
Bachelor’s Degree preferred. - A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is required.
- The Nurse Licensure Compact (NLC) allows a registered nurse (RN) and licensed practical/vocational nurse (LPN/LVN) to have one multistate license in a primary state of residency (the home state) and to practice in other compact states (remote states), while subject to each state’s practice laws and discipline.
- Current CPR required; CHPN preferred.
- Minimum 1-year related experience for inpatient units (IPU) and 2 years for other nursing positions. Experience in direct care of patients with life limiting illnesses required. Related experience may include hospice and palliative care; family medicine; internal medicine; geriatrics, oncology; home health care; medical surgical.
- Computer proficiency is required.
