What you will do:

• Assess and evaluate patients’ condition to create tailored rehabilitation plans for stroke recovery.
• Implement evidence‑based therapy interventions to improve patients’ functional abilities and autonomy.
• Collaborate with a multidisciplinary team to ensure the highest quality of patient care and optimal recovery outcomes.
• Monitor patient progress and adjust treatment plans as necessary to achieve therapy goals.
• Engage with patients and families to provide education and support throughout the rehabilitation process.

Requirements:

• Degree in Occupational Therapy with a post‑graduate qualification preferred but not essential.
• Enhanced community‑based stroke experience.
• Proficiency with Systm One is desirable, however not essential.
• HCPC registration is required.
• Must be a car driver with access to a vehicle, and comfortable with covering Hull and East Riding areas.
