In this role, you will support young people with learning disabilities as they transition from children’s services into adult services, helping them achieve positive outcomes while earning a competitive rate of £30 per hour.

• Carry out Care Act assessments and reviews to identify the needs of young people transitioning into Adult Social Care.
• Develop personalised Care and Support Plans that promote independence and positive outcomes.
• Manage a statutory caseload while ensuring all documentation and processes are completed accurately.
• Work closely with families, education teams, healthcare professionals, and partner agencies to provide holistic support.
• Advocate for young people and guide them through the adult social care system.
