Main duties of the jobWe are seeking an ACP with primary care experience who:
Manages acute and chronic illness confidently
Works autonomously while valuing collaborative team working
Supports the on-call GP within our digital triage hub
Contributes to service development within an innovative practice
About usWe are a total digital triage practice, successfully operating this model for the past three years. This allows us to ensure that patients are seen by the right clinician, in the right length appointment, based on clinical need, supporting both high-quality careand clinician wellbeing.

The Advanced Clinical Practitioner will work as an autonomous clinician within a GP practice operating a total digital triage model
. The post-holder will assess, diagnose, and manage patients with a range of acute and long-term conditions, ensuring patients are directed to the most appropriate care in a timely manner. The role supports safe, effective service delivery while contributing to service development and team working.
Key Duties and Responsibilities
- Independently assess, diagnose, and manage patients presenting with acute illness and long-term conditions
- Undertake clinical triage within the digital triage system, prioritising and directing patient care appropriately
- Provide clinical support to the on-call GP within the digital triage hub
- Prescribe medication in line with scope of practice, clinical guidelines, and local policies
- Make appropriate referrals to secondary care and community services
- Maintain accurate and timely clinical records using the practice clinical system
- Participate in clinical meetings, peer review, and multidisciplinary discussions
- Contribute to service development, quality improvement, and practice initiatives
- Support teaching, learning, and supervision of trainees and other staff as appropriate
- Work within professional, ethical, and legal frameworks, maintaining registration and revalidation requirements
Professional Responsibilities
- Practice in accordance with NMC / HCPC codes of conduct and professional standards
- Maintain up-to-date clinical knowledge and skills through CPD
- Work within practice policies, procedures, and governance frameworks
- Contribute to a positive team culture and support colleague wellbeing
Other Requirements
- Willingness to participate in training, education, and clinical meetings
- Ability to work flexibly to meet service needs
- Commitment to equality, diversity, and inclusion
- Qualified Advanced Clinical Practitioner with relevant MSc or equivalent
- Current NMC / HCPC registration (as appropriate)
- Significant experience working in primary care
- Ability to independently assess, diagnose, and manage acute presentations and long-term conditions
- Strong communication and interpersonal skills
- Commitment to high-quality, patient-centred care
- Team-player with a flexible, can-do attitude
- Willingness to contribute to teaching, learning, and clinical meetings
- Experience working within a total digital triage or similar access model
- Experience of triage
- Experience supporting or working alongside on-call GP services
- Interest in service development, innovation, or quality improvement
- System One Experience
