Job Title: Care Coordinator

Salary: £30,302.64 per annum
Working Hours: 37.5 hours per week
Working Pattern: Working Monday-Friday with flexibility to work evenings and weekends when required

Aboutyou
- Experience of working within a CQC registered supported living or extra care service
- Experience of leading a team, working in challenging environments with the ability to plan and prioritise tasks to achieve positive results
- Ability to review, monitor and evaluate outcomes in the delivery of a CQC registered service
- Competent IT skills
- Experience of producing reports, statutory submissions and other communications and analysing outcome data to understand and drive service improvements

Role Profile Lead a team- Create a safe team environment where colleagues can talk openly and honestly
- Handle administrative systems and procedures: staff deployment, rosters, medication, financial administration, etc.
- Hold regular one‑to‑one meetings with care team to reflect on achievements and promote good working practice
- Conduct six‑monthly performance reviews to support colleagues to fulfil their potential
- Facilitate regular team meetings to share ideas and best practice
- Use reflective practice techniques so the team learns from experience
- Build positive relationships with catering, facilities, cleaning and maintenance contractors to ensure residents receive seamless service
- Supervise the service to CQC and contractual requirements, including annual review of contract performance
- Promote and implement care policies and procedures to ensure safety and effectiveness
- Advise stakeholders of any changes to policies and procedures
- Participate in setting and managing service budgets, monitoring spend and reporting variances
- Meet Riverside performance targets
- Resource, plan and deliver colleague rota covering all shifts
- Implement and oversee robust medication procedure in line with national Safe Handling guidelines
