Overview

U.S.VETS is the largest nonprofit organization with boots on the ground to combat America’s veteran homelessness crisis. Our holistic approach delivers intervention and prevention services, including housing, mental health and career programs, and wraparound services. With residential and service sites nationwide, U.S.VETS helps veterans and their families transition to civilian life. We are developing housing across the country and expanding vital homeless prevention programs that provide post-9/11 veterans and families with career and mental health services.

Position:
Case Manager II

The Case Manager II reports to the Program Coordinator of the U.S.VETS program to which the Case Manager is assigned. This role provides guidance to veterans to help them achieve their highest level of independence. Case management is a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates the selection of services to meet each client’s needs.

Responsibilities

  • Assesses veterans referred to the program to identify problems, needs, or barriers in housing, employment, education/training, financial, sobriety, legal, family/social, and medical/psychiatric areas
  • Coordinates care with VA personnel and other community partners for eligibility documentation, mental and physical health services
  • Facilitates client intake process and compiles client files ensuring all program eligibility documentation is complete
  • Collaborates with clients to complete Individual Action Plans (IAP)
  • Maintains confidential client files in a professional and timely manner; ensures forms are current, complete, signed where applicable, and filed according to standards
  • Meets with clients as required and documents progress notes in DAP format
  • Reviews and updates IAPs monthly; completes timely discharge summaries
  • Conducts home visits to provide case management and ensure client health and welfare
  • Assists in quality management activities, data collection, and reporting
  • Facilitates client life skills classes and provides specialized case management services and life skills groups/classes to veteran subpopulations (e.g., female veterans, disabled veterans, Iraq/Afghanistan veterans)
  • Performs random drug and alcohol screenings as applicable
  • Maintains HIPAA-compliant confidentiality
  • Completes accurate data entry into HMIS and other required databases
  • Documents client referrals for services
  • Attends scheduled meetings and trainings; maintains time sheets, mileage logs, and other documentation
  • Drives company vehicle for client transportation or other work-related activities
  • Performs other duties as required

Additional Program-Specific Duties

U.S.VETS has various programs to which a Case Manager may be assigned. The following duties apply to program-specific assignments:

Case Managers with Transitional Housing Clients

  • Provide case management services that aim to transition clients to permanent housing
  • Account for clients daily through contact, bed check, and daily sign-in logs
  • Monitor program rent/participant fee compliance to support housing prioritization
  • For programs with scattered-site transitional housing, locate housing units that meet clients’ needs

Case Managers with Permanent Housing Clients

  • Focus on obtaining and maintaining permanent housing
  • For scattered-site housing, locate suitable housing for clients
  • Support families with dependent children by enrolling children in school and connecting to community services

Case Managers with Workforce Clients

  • Assist with program assessment and enrollment
  • Develop Individual Employment Development Plans; address barriers to employment and set short- and long-term goals
  • Provide individualized resume and cover letter assistance
  • Facilitate groups/classes (e.g., Job Placement, Job Retention, Employment Barriers, Computer Skills, Money Management)
  • Present clients’ employment backgrounds to prospective employers
  • Maintain knowledge of community employment resources and procedures

Case Managers with SSVF Clients

  • Conduct rapid rehousing and homeless prevention services
