Advanced Practice Nurse - CORHLT. The Harris Health System Advanced Practice Nurse - Correctional Health (APN- CH) functions as a clinical provider to socially marginalized patients under the jurisdiction of the justice system. The APN-CH evaluates, plans and provides care to patients in consultation with and under the direct supervision of the attending physician and serves as a clinical leader responsible for facilitating quality patient care in collaboration with pavilion leaders, nursing staff, and other disciplines.
The APN-CH provides both programmatic and individually-delegated patient care. The APN-CH serves as a clinical expert and consultant to support the healthcare system in providing patient-centered, culturally-competent, outcome- focused therapeutic care within the management of specified populations through independent and collaborative clinical management. This is done through intake health assessments, acute care visits, chronic care visits, and life saving measures to justice involved individuals of the Harris County Jail.
The APN-CH aligns practice with professional rules and regulations, accreditation standards, and organizational policies and procedures. Ensure organizational compliance with regulatory and accreditation requirements. The APN-CH provides personalized care and service consistent with the Harris Health System care values on people, diversity and inclusion, compassionate care, trust, integrity, mutual respect, communication, and education.
