Hospice Registered Nurse; RN Northwest, Southwest

Overview

At Pathways Hospice (Newest Start Up), our associates are our most valuable asset. Each associate provides a service that directly or indirectly affects patients, families, other team members, and the public. We strive to excel in providing the highest quality healthcare and customer service possible, which is incumbent upon the excellence and commitment of our leaders and caregivers.

We are located in Houston, TX (Harris County). Ultimately, we will serve the vast majority of Harris County initially; our service will start in the East and Southeast Region of Houston at the initial phase of service (Baytown, LaPorte, Clear Lake, etc.).

We are excited to contribute to this growth in the amazing healthcare direct-community with our newly formed company, contributing to all patients with a continuum of care and long-term care services. It is our goal to DELIVER excellent and patient experiences to every patient and resident family member.

Shifts Available:
Full Time, PRN

Service Areas with Openings:
Spring, Northwest & Southwest Houston

• Oversee nursing care team.
• Communicate and collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care.
• Plan, organize, and direct home care services.
• Conduct assessments of patient care needs and develop care plans.
• Educate patients and their families on managing their health conditions, medication regimens, and any necessary lifestyle changes.
• Prepare timely, complete, and accurate clinical notes.
• Night and Weekends are required per hospice needs of patients.

• Valid RN license in the State of Texas.
• Hospice experience (at least 1 year).
• Knowledge of home health regulations and general healthcare compliance.
• Experience with Home Care Home Base (HCHB) EMR (preferred but not required).
• Excellent communication and people skills are essential.
• Ability to work independently and as part of a team.
