Full Time Case Manager (Utilization Management)
Cypress Creek Hospital is a free-standing 128-bed acute care psychiatric hospital located in north Houston, TX, minutes south of The Woodlands. The hospital offers inpatient acute care, partial hospitalization, and intensive outpatient programs, serving over 10,000 patients each year. A dedicated team of psychiatrists, nurses, social workers, mental health technicians, and support staff delivers compassionate care as the hospital’s mission.
ResponsibilitiesThe Case Manager collaborates with physicians to coordinate hospital, patient, and community resources, providing optimal care and securing appropriate reimbursement authorization. Hours are based on departmental and facility needs and may include day, evening, and weekend shifts as directed by the Intake Director or CEO/Managing Director.
Job Duties / Responsibilities- Advocate for the patient, maximizing cooperation between patient, patient family, treatment team, and managed care organizations.
- Collaborate with the treatment team, business office, and admissions to ensure that patient needs are met first.
- Deliver information to the business office, admissions office, managed care organizations, and the director of utilization management while monitoring documentation quality to facilitate collections.
- Communicate pertinent information internally in a timely manner and provide current certification status to the treatment team and physician.
- Demonstrate knowledge of referral sources.
- Facilitate all patient transfers as needed.
- Identify and assess available resources, communicating options to the physician and treatment team.
- Serve effectively with patients, families, managed care organizations, referral sources, community resources, departmental colleagues, facility co-workers, and the management team.
- Interact with patients in a professional manner using professional boundaries.

Qualifications- Bachelor’s degree in social work, mental health, or nursing preferred; a focused degree, licensure, or relevant experience is acceptable.
- Knowledge of behavioral health systems and utilization management required.
- Minimum two years professional experience in a health care setting, preferably inpatient; minimum one year of utilization management or case management experience required.
- LPC, LMSW, LVN, or RN license preferred.
- CPR and HWC crisis intervention training must be completed prior to beginning work (can be obtained upon hire).
