This role is ideal for a licensed Registered Nurse (RN) with strong leadership experience in home health, hospice, or clinical operations management.

Key Responsibilities- Direct the day-to-day operations of the home health branch, ensuring compliance with all applicable federal, state, and accreditation standards (CMS, Medicare, Joint Commission, etc.)
- Oversee the coordination of patient admissions and discharge planning to ensure a seamless patient experience
- Provide clinical oversight and leadership to interdisciplinary home health staff, including RN Case Managers, LPNs, therapists (PT, OT, ST), aides, and administrative personnel
- Interview, hire, train, supervise, and evaluate staff while fostering a supportive and collaborative team culture
- Promote best practices in skilled nursing, chronic disease management, and patient-centered home care
- Monitor and improve operational efficiency, service delivery, and patient satisfaction
- Lead performance management efforts, including staff evaluations, scheduling, workload distribution, and addressing employee concerns
- Ensure continuous survey readiness and participate in quality assurance and performance improvement (QAPI) initiatives
- Manage branch financial performance, including budgeting, payroll, supply costs, and referral development
- Build and maintain strong relationships with physicians, hospital discharge planners, and community referral sources

Qualifications – What You ll Bring- Current RN license in good standing in the state of practice.
- Associate s degree in Nursing or related field.
- 2+ years of experience as a Home Health Administrator, Director of Operations, or equivalent leadership role.
- Strong knowledge of CMS regulations and home health compliance requirements.
- Proven leadership, communication, and problem-solving skills.
- Experience as an interim or turnaround leader in home health.
- Bachelor s degree in Nursing or related field.
- Background in managing multi-site or high-volume agencies.
- Track record of improving operational metrics in a short time frame.
