A Hug Away Healthcare is seeking a Nurse Practitioner (Wound Care) to join our compassionate and dedicated team. We specialize in home health, pediatrics, and hospice care—providing skilled, personalized services wherever our patients call home.
Primary functions include overseeing the clinical care and treatment of patients and families; consulting with the patients' attending physician, in consultation with the hospice physician; and providing clinical direction to the Interdisciplinary Group/Team to ensure quality care.
Job Qualifications:
- Education:
Graduate from a Baccalaureate School of Nursing, with a Nurse Practitioner (NP) focused Master's or Doctoral Nursing Program. Current state license as a Registered Nurse and as an Advanced Practice Nurse. Current driver's license. Two years' experience as an Advance Practice Nurse in a clinical care setting preferred; hospice or home health experience preferred, in-home Wound Care experience required. - Experience working with ordering and applying wound care products after grafting.
- Nursing skills as defined by generally accepted standards of practice. Comprehensive knowledge of pharmacology, palliative care, and end-of-life care. Good interpersonal skills.
Transportation:
- Reliable transportation and valid auto liability insurance.
Environmental and Working Conditions:
- Works in patients' homes in various conditions; possible exposure to blood, bodily fluids, and infectious diseases.
- Ability to work a flexible schedule; ability to travel locally; some exposure to unpleasant weather; PRN emergency calls.
Physical and Mental Effort:
- Prolonged standing and walking required, with ability to lift up to 50 lbs and move patients. Ability to work under stressful conditions to meet deadlines and patient needs, make quick decisions, and provide resource acquisition; meet patient and family psychosocial needs.
- Collaborates with the IDG/T to ensure appropriate evaluation and palliative treatment of terminal disease.
- Consults with and assists in the development of patient/family care plans to ensure quality care.
- Educates patients and families regarding disease processes, self-care techniques, and end-of-life care. Provides training to staff as needed.
- Performs and documents visits based on the care plan and patient/family needs.
- Interacts with physicians regarding palliative care, in collaboration with hospice physician.
- Provides education to IDG/T members on clinical issues related to palliation.
- Participates actively in the IDG/T, communicating effectively and timely.
- Informs physicians of changes in patient needs and outcomes of interventions, and evaluates patient/family responses.
- Completes face-to-face encounters with patients as scheduled and provides clinical assessment results to hospice physicians.
- Uses standard precautions and adheres to hospice policies regarding protected health information.
- Maintains current knowledge in palliative medicine and related fields.
- Provides on-call services to patients and families as assigned.
