Registered Nurse (RN) Home Health – Crosby, Highlands

Join Transition Home Healthcare, an elite home health agency serving Houston, TX and surrounding areas since 2010. We are looking for a dedicated Registered Nurse (RN) to provide high‑quality care in the home setting.

Shift Available: PRN

Service Area for Coverage: Crosby, Highlands

• Oversee nursing care team.
• Communicate and collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care.
• Plan, organize, and direct home care services.
• Conduct assessments of patient care needs and develop care plans.
• Educate patients and their families on managing their health conditions, medication regimens, and necessary lifestyle changes.
• Prepare timely, complete, and accurate clinical notes.

• Valid RN license in the State of Texas.
• At least 1 year of home health experience.
• Knowledge of home health regulations and general healthcare compliance.
• Experience with Home Care Home Base (HCHB) EMR preferred.
• Excellent communication and people skills.
• Ability to work independently and as part of a team.
