About the Role:

The Licensed Vocational Nurse (LVN) plays a critical role in providing compassionate and skilled nursing care within a long-term skilled nursing facility. This position focuses on delivering high-quality patient care to residents with chronic illnesses, disabilities, or recovery needs, ensuring their comfort, safety, and well-being. The LVN collaborates closely with registered nurses, physicians, and other healthcare professionals to implement individualized care plans and monitor patient progress.

Through meticulous documentation and adherence to healthcare regulations, the LVN helps maintain the facility’s standards of care and compliance. Ultimately, the LVN contributes to enhancing the quality of life for residents by addressing their physical, emotional, and medical needs in a respectful and professional manner.

Minimum Qualifications:
  • Valid and current Licensed Vocational Nurse (LVN) license in the state of employment.
  • Completion of an accredited vocational nursing program.
  • Experience working in a long-term skilled nursing facility or similar healthcare setting.
  • Basic Life Support (BLS) certification.
  • Strong understanding of nursing principles, patient care standards, and healthcare regulations.
Preferred Qualifications:
  • Experience with electronic health record (EHR) systems.
  • Additional certifications such as IV therapy or wound care specialty.
  • Previous experience working with geriatric or chronically ill patient populations.
Responsibilities:
  • Administer medications and treatments as prescribed by physicians and according to facility protocols.
  • Monitor and record patients’ vital signs, symptoms, and overall health status regularly.
  • Assist in developing and updating individualized care plans in collaboration with the healthcare team.
  • Provide wound care, catheter care, and other nursing procedures as required.
  • Communicate effectively with patients, families, and interdisciplinary team members to ensure coordinated care.
  • Maintain accurate and timely documentation of all nursing activities and patient responses.
  • Ensure compliance with infection control standards and safety regulations within the facility.
  • Support patients with activities of daily living, including mobility, hygiene, and nutrition as needed.
Skills:

The required skills in long-term skilled nursing facilities are essential for delivering consistent and effective patient care, including administering medications, monitoring patient conditions, and performing nursing procedures. Proficiency in these skills ensures that the LVN can respond promptly to changes in patient health and provide appropriate interventions. Preferred skills such as experience with electronic health records enhance the efficiency and accuracy of documentation and communication within the care team.

Additional certifications like wound care or IV therapy allow the LVN to perform specialized tasks that improve patient outcomes. Strong communication and interpersonal skills are vital for building trust with patients and collaborating effectively with multidisciplinary teams to provide holistic care.
