We’re looking for a Hospice RN – Regional Support to join our team. In this dynamic role, you’ll provide direct care to patients and support to families, while also serving as a floating clinician—temporarily assigned to different hospice branches based on census needs or staffing gaps. If you’re energized by new settings, adaptable to change, and passionate about purpose-driven care, this role could be a perfect fit.
Responsibilities- Deliver high-quality, compassionate nursing care for hospice patients.
- Manage and implement individualized plans of care.
- Support and lead interdisciplinary team members (LPNs, CNAs, HHAs, etc.).
- Assess and address patients’ physical, emotional, spiritual, and environmental needs.
- Accurately document all care provided and ensure compliance with hospice protocols.
- Educate families and caregivers on care instructions, medications, and safety.
- Participate in the on-call rotation and provide after-hours care as needed.
- Collaborate with physicians, referral sources, and the care team.
- Assist with referrals, regulatory documentation, and payer compliance.
- Travel to various Gentiva hospice locations as assigned to fill urgent needs.
This role typically follows a Monday–Friday schedule, with occasional on-call responsibilities. Travel is required and may include:
- Option 1:
Overnight travel to cities within a 100-mile radius. - Option 2:
Frequent overnight travel to multiple states. - Option 3:
Regional travel with overnight stays based on assignment needs.
A company fleet car is provided for eligible employees.
Qualifications– What You’ll Bring
- Current, unencumbered RN license in the state of employment.
- Minimum of 1 year of RN experience (hospice, home health, acute care, long‑term care, oncology, or other high acuity setting).
- Current CPR certification.
- Valid driver’s license, auto insurance, and reliable transportation.
- Hospice and end‑of‑life nursing experience.
- Skilled in clinical assessments and compassionate communication.
- Strong critical thinking, flexibility, and problem‑solving skills.
- Comfortable working in varied environments with shifting needs.
- Proficient in documentation and electronic health records.
- Calm and supportive in crisis situations; skilled at family engagement.
