Trumen Physicians + Associates is contracting with an experienced and compassionate Nurse Practitioner to collaborate with our Trumen Transitional Care Team (TTC) on a part‑time, supplemental basis in North Houston, TX. Trumen’s Transitional Care Program supports adults with acute or chronic conditions as they move from the hospital or skilled nursing facility back to their community setting—such as their home or assisted living. The goal is to prevent avoidable readmissions by ensuring a smooth, medically supported transition.

• Travel to patients’ homes shortly after discharge to provide in‑person follow‑up care
• Reconcile medications and review treatment plans within your scope of practice
• Manage acute and chronic conditions in collaboration with Trumen‑affiliated physicians and the patient’s care team
• Communicate with patients’ families and primary‑care providers as needed to support continuity of care

Coordinate with a dedicated TTC Coordinator to organize your monthly schedule based on your availability and travel preferences.

• Conduct home visits for patients recently discharged from hospitals or skilled nursing facilities
• Perform medication reconciliation and health assessments within your scope of practice
• Provide short‑term management of chronic and acute conditions in collaboration with Trumen‑affiliated physicians and the patient’s care team
• Educate patients and families on care plans and red‑flag symptoms
• Coordinate care and updates with primary providers and specialists as needed to ensure continuity of care
• Complete documentation accurately and in a timely manner using EMR

• Active Texas Nurse Practitioner license
• Minimum of 1 year of NP experience (transitional care, home health, geriatrics, or post‑acute preferred)
• Strong interpersonal and critical‑thinking skills
• Comfort working independently in a home‑based setting
• Reliable transportation throughout North Houston
