LVN Charge Nurse (Friday‑Sunday) 12hrs – Houston – Woodway Community – Tradition Senior Living, LP
OverviewTradition Senior Living is a Texas-based luxury senior living organization committed to excellence in all we do to serve our residents. Each community offers a comprehensive continuum of care, including Independent Living, Assisted Living, and Memory Care.
Responsibilities- Full time: 7 a.m. – 7 p.m. (Friday‑Sunday shift)
- Administer and supervise medications as needed.
- Obtain and record residents’ vital signs, weight, food and fluid intake and output.
- Observe residents to detect responses to medications and report or notify the Healthcare Director of any unexpected reactions.
- Communicate with physicians’ offices to manage or change orders for residents.
- Ensure emergency response system calls are answered in a timely manner to attend to residents’ needs immediately after a call is signaled.
- Attend all required training and conferences to stay abreast of changes in the nursing profession and maintain appropriate CEU’s for licensing.
- Ensure all direct‑care staff duties are performed as assigned and documented appropriately.
- Participate in the care‑planning process as assigned.
- Assist in training direct‑care staff to meet state regulation requirements, as directed by the Healthcare Director.
- Address concerns of team members, families, and residents to promote satisfaction and good communication.
- Maintain confidentiality of all pertinent personal or health information concerning residents and staff.
- Perform other duties as assigned to ensure the success and safety of the Tradition residents and team members.
- Minimum of one year experience in home health, assisted living, hospital, or long‑term care environment.
- Current state license as a Licensed Practical Nurse / Licensed Vocational Nurse (LPN/LVN).
- Management experience preferred.
